This innovative project provides a proactive approach to diabetes and chronic disease management in South Asian and Chineseindividuals and families at risk for and/or living with diabetes and other chronic diseases such as cardiac illnesses.
The goal of DWTC is to provide appropriate service information and more seamless referrals and navigation supports for seniors and caregiver living in the community.

Aim: To ensure that traditional links between community elders and youth are maintained and enhanced. To develop a catalogue of activities that will support these relationships and roles.
The Central Local Health Integration Network (CLHIN) has approved aSouth Asian Adult Day Program, through their Aging at Home Strategy.
This is a partnership program between Unionville Home Society and Social Services Network.
The South Asian Adult Day Program will deliver an integrated(cognitive/frail elderly) ethno-specific program in a culturally andlinguistically sensitive manner. It will take place two days per week(Saturdays and Sundays) at Unionville Home Society, in Markham and willbe delivered by staff speaking Hindi, Urdu, Punjabi, Gujarati and Tamil.
The main purpose of the project is to encourage and enable South Asian seniors and youth
to exchange experiences, skills, resources and wisdom through planning,
developing, and executing various intergenerational activities.
